• Please note we cannot issue Covid recovery cert’s as these can only be issued by the HSE

• Once you’ve completed your appointment booking below, our Patient Care team will contact you within 60 minutes to confirm your booking.

• This service is currently not available for GMS (Medical Card) patients. If you are a GMS patient, please visit Doctor On Duty to request a GP Appointment

• This service is not available to K-Doc, D-Doc, North Doc patients.

• Please note that we do not issue social welfare forms or social welfare sick certs

• Remember: if you feel your condition is an emergency, you should call an ambulance (999) immediately.
